By checking & upon submission of this application, I hereby waive and release all rights and claims for damages I may have against 308 United Softball and its volunteers on account of any injuries or illnesses sustained by my child while attending the skills camp/tryout. I authorize the director of the organization or his designee to select hospital facilities and/or physicians of his choice and authorize treatment on an emergency basis if such treatment becomes necessary as a result of participating in the 308 United Softball Skills camp. Pictures from the camp may be used in promotional materials for 308 United Softball either in print or on the website.

I understand that $200 tryout fee must be paid IN ADVANCE and will be reimbursed by 308 United if my child does NOT make a team. An additional $200 will be due at a later date if my child is placed on a team.
